Map & Move Waiver

Thank you for booking an experience with Map&Move, LLC. These Terms and Conditions (the “Terms”) apply to any travel products and/or services you have booked with or purchased from Map&Move, and govern the contractual relationship between you and Map&Move with respect to any products and/or services (including trips, events, classes) that are hereinafter referred to as “Experience”).

To the best of my knowledge, I am in good physical condition and fully able to participate in all trips led by Map&Move, LLC. I am fully aware of the risks and hazards connected with the participation in events, including physical injury or even death, and hereby elect to voluntarily participate in said events, knowing that associated activities may be hazardous to me and to my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or loss or damage to property owned by me, as a result of participation in these trips.

I understand that Map&Move, LLC. is not responsible for my safety and knowingly and voluntarily agree to assume any and all risks associated with participation in the above-described opportunity (the “Program”). I also acknowledge that my participation in the Program is optional and that my participation would not have been permitted without this waiver and release. I understand and acknowledge that MAP&MOVE is not providing personal chaperones or supervision with respect to the Program, and that I am responsible for my individual conduct, health and safety at all times.

I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO HOLD RESPONSIBLE OR SUE Map&Move, LLC. and organizers from any and all liability, claims, demands, action and causes of actions whatsoever arising of or related to any losses, damage or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in physical activity, or while on or upon the premises where the events are being conducted.

It is my expressed intent that this release and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a RELEASE, WAIVE, DISCHARGE, and CONVENTION TO SUE the above named RELEASEES. I hereby hold harmless and release and forever discharge Map&Move, LLC. and organizers from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be constructed in accordance with the laws of the State of New York.

I also authorize Map&Move, LLC  permission to use my likeness in a photograph or video in any and all of its publications. I understand and agree that any photograph or video using my likeness will become property of Map&Move, LLC. and will not be returned. I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears and acknowledge that photographs or videos may be edited, altered, copied, exhibited, or distributed for purposes of publicizing Map&Move, LLC.’s programs or for any other related, lawful purpose. I acknowledge that since my participation with Map&Move, LLC. is voluntary, I will receive no financial compensation.

I fully understand the contents, meaning, and impact of this release. In agreeing to this release, I acknowledge and represent that I HAVE READ THE FORGOING Waiver of Liability and Hold Harmless Agreement, UNDERSTAND IT AND AGREE TO IT VOLUNTARILY as my own free act and deed; no oral representations, statements or inducements, apart from the foregoing written agreements have been made and I EXECUTE THIS RELEASE FOR FULL, ADEQUATE AND COMPLETE CONSIDERATION FULLY INTENDING TO BE BOUND BY SAME.
